Pats For Patriots - Saluting Massachusetts Veterans 11.11.2012

The "Pats For Patriots" program is a partnership between the MBCR/Military Friends Foundation honoring Massachusetts veterans. This Veterans Day, "Pats For Patriots" will send a group of local veterans to the New England Patriots game on the Gold Star train. Veterans, Service Members and Families of the Fallen, are invited to apply below. Applications will be accepted until October 31, 2012. Winners will be randomly selected and notified on or before November 5th. Tickets will be distributed at South Station 45 minutes before the Gold Star train boards for the game. Only one application per Veteran, Service Member or family of the Fallen will be accepted.
  • Those selected to receive tickets will need to provide a DD214, LES or other proof of military service. Military Friends Foundation will work with families of Fallen Service Members.
    PARTICIPANT RELEASE OF LIABILITY AND IMAGE RELEASE (PLEASE READ CAREFULLY AND THOROUGHLY) I agree that any and all representations made and releases, waivers covenants, consents and permissions given by me hereunder are given on behalf of me and any and all of my minor children or persons over whom I have guardianship participating in or attending the Military Friends Foundation/MBCR/MBTA “Pats for Patriots” event. By submitting this application, on behalf of myself and my minor children or persons over whom I have guardianship, along with any and all respective heirs, agents, assigns, representatives, servants, executors, administrators, and successors, to waive the Military Friends Foundation, MBCR, and MBTA arising out of, or relating in way, to the participation in or attendance at said "Pat for Patriots" event and hereby consent to the use of my/our/their likeness and image arising out of participation in or attendance at said "Pat for Patriots" event. I understand this event is a public event.
